Payment Terms: Total fee agreed upon by client and Starz DJ Entertainment is $__________, tax included. Star DJ
Entertainment is holding a tentative reservation for client's event until the above "response date". To confirm this agreement,
return one signed and completed copy along with a deposit of $50.00 before the above response date. Agreements received after
the response date are subject to availability. If sending a deposit, it may be paid no later than two weeks prior to the event by
check or cash. The remaining payment must be made in full prior to performance in cash or check. In the event of non
payment prior to the performance, the client agrees this agreement shall be cancelled immediately with no performance required
and any previous payments shall be forfeited to Starz Entertainment. In the event of non-payment after the performance, client
agrees to remit any uncollected balance including reasonable collection or attorney fees.

Cancellation: This agreement cannot be canceled or modified except in writing by either the client or Starz DJ Entertainment. If
the cancellation is initated by client less than 60 days prior to the event, then the deposit will be forfeited. For cancellations less
than 30 days prior to the event client is responsible for the total fee. Client can reschedule, only by signing a new agreement for a
substitute engagement preformed in the following 6 months. Rescheduled events are subject to availiablity.

Provisions: The client shall ensure that: (1) Facility provides dj with 12 wide x 6 deep (feet) area within 25 feet of four
110-volt (3 prong grounded) outlets (2) Facility is open at least one and a half hours prior to start time. (3) Facility meets all
federal and state regulations and has all the appropriate music licenses and permits. (4) Resonable steps will be taken to
protect rising stars equipment, personnel, music and crowd control will be provided if warranted. (5) For outdoor events,
shelter is provided that covers rising stars equipment from adverse weather conditions. Furthermore client accepts full
responsibility and is liable for any damages, injuries, or delays that occur as a result of failure to comply with these
provisions. In the event of circumstances deemed by dj to present a real or implied threat of injury or harm to dj, equipment,
or recordings, then dj reserves the right to cease performance until such time as client resolves the threating situation. In the
unlikely event the dj performance is delayed, liability is limited to providing client with performance time equal to time
lacking.

Disclaimer: This agreement to perform shall be excused by detention of personnel by sickness, accidents, riots, strikes, epidemics, acts
of God, or any other legitiment condition beyond Starz DJ Entertainment's control. If such circumstances arise, all reasonable efforts
will be made to find comparable replacement entertainment at the agreed upon fees. Should Starz DJ Entertainment be unable to procure
a qualified replacement, client shall receive a full refund of all prepaid fees. Client agrees that in all such circumstances, Starz DJ
Entertainment's liability shall be exclusively limited to refunding the fees paid and Starz DJ Entertainment shall not be liable for indirect
or consequential damages arising from breach of contract. Furthermore client agrees to defend, indemnify, assume liability for and hold
Starz DJ Entertainment harmless from any and all claims, demands, damages, losses, suits, proceedings, penalties, expenses, or other
liabilities including attorney fees and court costs, arising out of or resulting from the performance of this contract, regardless of basis (
except for gross negligence on the part of Starz DJ Entertainment )
